Brainstorm requirements spanning multiple concerns are hard to scan as
a flat list. The skill now groups them under bold topic headers while
keeping global R# IDs for compact cross-referencing.

Also teaches the coherence reviewer to detect and auto-fix ungrouped
multi-concern requirements, and fixes a batch_confirm/auto
classification contradiction in the findings schema where deterministic
summary corrections were incorrectly classed as batch_confirm.
