Conal Mullan 3c3bf68cf9 DOCS: move 'Using with Codex' details to docs/codex.md

The README section had grown to ~55 lines, mostly a hardcoded list of 26
installed skill directories that drifts stale as skills are added (it was
already missing ideogram4). The script discovers skills dynamically from
.claude/skills/, so docs/codex.md now explains the discovery rule instead
of enumerating directories. README keeps a three-line summary: the command,
a link to the doc, and the contributor credit.
