Conal Mullan 9c6a484437 FIX: Codex migration name uses registry, drop no-op skip entry (closes #20) ...

- Read toolkit name from _internal/toolkit-registry.json with fallback to
  repo_root.name. Descriptions now correctly say "in claude-code-video-toolkit"
  regardless of the local checkout directory.
- Remove the openclaw-video-toolkit skip_skills entry that never fired
  (.claude/skills/ is the only path scanned by design). Brief comment in
  load_skill_specs explains why so future readers don't "fix" the missing scan.
