fix(hooks): narrow simple-notifications to Stop and Notification events
...
The PostToolUse hook with a wildcard matcher fires on every single tool
call, flooding the user with dozens of notifications per turn. Replace
with Stop (fires once when Claude finishes) and Notification (fires
when Claude needs user input), matching the pattern already used by
discord-notifications and slack-notifications hooks.

fix: recognize both "Task" and "Agent" tool names in analytics ( #491 )

Claude Code JSONL logs use `content.name === 'Agent'` but the
analytics code only checks for `'Task'`, causing Agent Usage
Distribution, Agent Activity Timeline, and Workflow Efficiency Score
charts to show empty data.
Accept both tool names in AgentAnalyzer, chats-mobile, and
teams-dashboard.

fix: remove shorthand model names causing 404 API errors ( #453 )

Remove `model: sonnet/haiku/opus` from 396 component files. The Anthropic
API requires full model IDs (e.g., claude-sonnet-4-6), so shorthand names
cause 404 errors. Removing the field lets Claude Code use the user's
configured default model instead.
