refactor: migrate worktree commands from wt/* to claude/* branch prefix
...
All worktree commands, skills, and guides now use claude/* as the default
branch prefix, matching Claude Code's actual worktree behavior. Updated
both .claude/commands/ and cli-tool/components/ versions.
