Docusign Integration Keys are environment-specific, so demo/developer
accounts must point at https://mcp-d.docusign.com/mcp. Reintroduce the
DOCUSIGN_MCP_URL variable (Salesforce's SALESFORCE_MCP_URL pattern) so
the endpoint is set at configure time instead of hardcoding the
production URL in mcp.json, and restore the matching README and
changelog wording.
