Replace claude-code-action (which doesn't support push events) with
openai/codex-action in the changelog-to-docs workflow. Codex runs on
push to next, updates docs, and creates a PR with the changes.
Add AGENTS.md as a tool-agnostic version of CLAUDE.md that any AI
coding agent (Codex, Claude, etc.) can use for repo context.
