Colby McHenry b653688f1b chore: add session handoff notes and update docs to reflect explore/node tools 
Adds three Claude session handoff documents capturing the cross-language impact coverage campaign, per-language coverage work, and issue triage session. Also removes references to deprecated `codegraph_context` and `codegraph_trace` tools from docs, replacing with `explore` and `node` trail terminology.
