Railly df266409a3 chore: remove unused issue template directory 
Skill-request issue template has zero usage. Killing it to keep the
repo lean. Skills get added by maintainers based on actual demand
signal, not GitHub issues.

Railly 267c3fc01e refactor: restructure to flat task-based skill architecture 
- Flatten plugins/clerk/skills/ to skills/ at root
- Add gotchas.md to each skill for common pitfalls
- Fix RBAC patterns to use has() instead of orgRole comparison
- Fix org middleware to check auth before org context
- Remove clerk-errors skill (not ready)
- Update templates for Next.js 15/16 proxy.ts convention
