Boden Fuller 8ec94093ce enforce two-tier orchestrator/worker fork classification 
Remove `context: fork` from orchestrator skills (evolve, crank, post-mortem) so
they stay in the main session with full visibility. Add `context: fork` to
codex-team (worker spawner). Document the two-tier rule in SKILL-TIERS.md and
skill-structure.md.

Rule: orchestrators stay in context, workers fork.

Also includes: evolve compaction-proof cycle recovery, rpi_loop improvements,
GOALS.yaml updates, and validation scripts.
