Dennis Jin f92103e386 Extract MCP server to separate package and rename _package-export to package ...

- Move server code (MCP, HTTP, SQLite) to new `mcp/` workspace package
- Remove server dependencies from main agentation package (now frontend-only)
- Rename `_package-export/` directory to `package/`
- Add `pnpm mcp` command to run MCP server from root
- Update all workspace references and build configs
