Adds webhookUrl prop to Agentation component that sends POST requests
on annotation events (add, delete, update, clear, submit). Includes
settings toggle to enable/disable webhooks at runtime.

- Move server code (MCP, HTTP, SQLite) to new `mcp/` workspace package
- Remove server dependencies from main agentation package (now frontend-only)
- Rename `_package-export/` directory to `package/`
- Add `pnpm mcp` command to run MCP server from root
- Update all workspace references and build configs
