Add `plannotator archive` subcommand and archive sidebar tab for browsing
saved plan decisions from ~/.plannotator/plans/. Plans show approved/denied
badges and timestamps. In-session mode uses the linked doc overlay pattern
so users can reference old plans while reviewing a current one.

- New archive server (packages/server/archive.ts) following annotate pattern

Save plan versions to ~/.plannotator/history/{project}/{slug}/ on every
plan arrival. Serves previousPlan and versionInfo via /api/plan, and adds
GET /api/plan/version?v=N for fetching any historical version. Enables
future diff UI. Deduplicates identical resubmissions.

* refactor: rename misleading "diff" terminology to "annotations" in plan review context

The plan review flow used "diff" naming (exportDiff, diffOutput, .diff.md,
"Raw Diff", "Download .diff") for what is actually user annotations/feedback
on a plan. This was confusing since the code review flow legitimately uses
"diff" for actual git diffs. Renames all plan-review-context "diff" references
to "annotations" across code, UI labels, file extensions, and docs.

* Add configurable agent switching for OpenCode

Add a Settings UI option to configure agent switching behavior after plan
approval. Users can now choose to switch to the "build" agent (default) or
disable agent switching entirely. This is particularly useful for oh-my-opencode
users who have their own agent switching logic.

Changes:
- Add agentSwitch.ts utility for storing agent switch preference
- Add "Agent Switching" dropdown in Settings UI
- Pass agentSwitch setting through /api/approve to OpenCode plugin
- Conditionally perform agent switch based on user preference
