Adds a "How it works" section covering the four-stage pipeline: ingest, extract, resolve, schema. Explains schema-on-the-fly coherence, supported file types, and the manifest-based delta system.
Obsidian uses filename as graph node label. _project.md made every
project show up as "_project" in graph view. Convention is now
projects/<name>/<name>.md. Updated all three ingest skills.
